Some insurance Plans cover Acupuncture

Here’s what we need to know to find out:

Copy of your insurance Card or Cards, front and back (a photo is fine).

Medicare generally doesn’t cover but we need that information if you have coverage.

Your address, phone number and birthdate. If someone else is card holder we need their name and birthdate as well.

Excel Medical Billing will determine coverage and when covered will bill the insurance directly. You are responsible for co-pays, deductibles and co-insurance.

Fax a copy of the card front and back along with your form to: FAX # 301-260-2838 or scan the card and send us an email. You can also call Excel Medical Billing at 301-570-9700.

Payment is requested at the time of treatment unless or until insurance is confirmed.
